AN
ENTRY FORM MUST BE COMPLETED WITH APPROPRIATE SIGNATURES FOR ALL HORSES
ON THE GROUNDS.
On Line entry does not relieve the requirement to have the assumption
of risk, waiver, and indemnification clause on the Entry Form signed
by the Owner/Agent and the riders, or if minors, the parents/guardians,
and on file at the horse show office before releasing competitor numbers.

Provide
the show office with photocopies of relevant 2008 Sports
License/Membership cards and the front page of Equine Canada horse passports. We do not have copies provided other horse shows. |
Provide the show office payment by cheque/Visa/Mastercard, or, provide an open signed cheque/Visa or Mastercard slip or provide a credit card number, expiry date, and name of cardholder |
After completing the following On Line Entry Form, double checking the input, and clicking SUBMIT ENTRY button at the bottom of the form, you will receive a Thank You, with a summary of your completed Entry Form. We recommend you print this for your records. You may then hit your "Back" key to return to the Form to allow further entries. You may fully clear the Form by clicking on the Reset button at the bottom of the Form or just make the needed modifications for each subsequent entry.
